Site Plan Review Application Form
A Site Plan application consists of a completed, signed form plus all drawings and written materials required by the Site Plan Review article of the Land Use Ordinance. The link below will take you to our portal where you can submit your application through the box titled "Subdivision & Site Plan Review Application". At the bottom of the online form is an "Upload Files" button where you can attached any required plans, diagrams, documentation, etc. Please review the process and checklist documents listed below to ensure you have provided all the necessary information to the Planner.

Subdivision Application Form

- Minor Subdivision requires Sketch Plan and Final Plan reviews.
- Major Subdivision requires Sketch Plan, Preliminary Plan, and Final Plan reviews.
